APM is looking for an Aged Care Assessor to carry out in-person assessments for older Australians and link them with essential services. You will be part of a multi-disciplinary team and receive a comprehensive induction program to set you up for success.

The successful candidate will have at least 1 year of aged care experience, excellent communication skills, and the ability to navigate online systems for effective service delivery within a team.
