The Care Coordinator is an advanced practice role, which serves as a conduit between the patient, nominated carer, the treating clinical team and the Psychiatrist, providing clinical care and coordination for the patients allocated to their VMO(s) across all service settings. There will also be opportunities to work within the Admissions and TMS teams. As an integral member of the hospital management team, responsibilities include assessment and screening, collection of collateral information from referrers and family, goal setting and discharge planning, education sessions with the patient and their nominated carer.



This role also provides patient follow up post discharge to promote and facilitate reintegration into the community.

Essential Criteria:

- Registration as a Registered Nurse with AHPRA and a current annual practicing certificate

- Post graduate qualifications in mental health

- Minimum of five years clinical experience in mental health

- Credentialed with Australian College of Mental Health Nurses

- Minimum of five years clinical experience in a mental health setting

- Excellent communication skills; ability to work collaboratively with consumers, carers, staff, hospital accredited professionals and members of the public

- Comprehensive computer literacy

- Excellent documentation and report writing skills

Desirable Criteria

- Problem solving and triage skills

- The ability to coordinate and manage a number of concurrent projects

- Self-starter, able to work autonomously

- Display a high level of customer service and professionalism
