CARE PARTNER - SUPPORT AT HOME
Ongoing Position 0.8 FTE (neg)– Based in Shepparton

Are you passionate about supporting older people to live safely, independently, and with dignity in their own homes? MHA Care is seeking a compassionate and skilled Care Partner to join our Support at Home team in a permanent part-time capacity.

This role is perfect for someone who thrives on building relationships, navigating services, and making a meaningful difference every day. You’ll work closely with clients, families, service providers, and the broader community to ensure each person receives high-quality, tailored support.

About the Role

As a Care Partner, you will:

- Work collaboratively with older people to identify their goals and develop person-centred care plans.

- Coordinate services, monitor budgets, and ensure supports align with consumer direction and safety.

- Advocate for clients and uphold their rights, ensuring quality standards are met.

- Build solid relationships with internal teams, external providers, and community networks.

- Respond to enquiries, promote MHA Care services, and support the ongoing improvement of our programs.

Essential Criteria

To be successful in this role, you will have:

- Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience.

- Demonstrated experience in care coordination, case management, or similar roles within aged care, health, or disability services.

- Solid understanding of the Support at Home Program, Aged Care Quality Standards, and the rights of older people.

- Strong communication, empathy, and cultural competence, with the ability to work respectfully with diverse individuals.

- Networking and liaison experience within the aged care and disability sectors, including negotiation and conflict resolution skills.





- Well-developed interpersonal skills, with the ability to discuss and resolve issues while promoting MHA Care’s services.

- Advanced computer skills, including Microsoft Office, email, and web-based systems.

- Current satisfactory Police Check.

- Current valid Driver’s License. Must be able to travel to clients outside of Shepparton and attend occasional office meetings in Yarrawonga.

Desirable

- Experience working in a rural or regional setting

- Diploma of Nursing

- Knowledge of Home Care Package/Support at Home financial management

- Familiarity with client management systems or care management software
