Care Connect is seeking a Care Manager for its Out of Hospital Care (OHC) program in Bella Vista on a full time, maximum-term basis (15 months parental leave cover). You will assess, plan and manage services to help clients safely return home from hospital while promoting independence and client choice.

You will develop person-centred care plans, build local referral networks and ensure high-quality service delivery at every stage of the client journey, with a solid, values-driven team culture.
