Uniting Care Queensland is seeking a Supported Living Facilitator to provide in-home care for clients in the community, supporting independence and improving quality of life. You'll spend your days on the road visiting clients in their homes and carrying out activities per their Care Plans.
Hours are mainly Monday to Friday 9am–5pm. Must have CPR/First Aid, a current driver's license and access to a vehicle, and be willing to drive between clients.
