Are you an experienced Support Coordinator who genuinely enjoys helping people navigate the NDIS, build strong support networks and work towards meaningful goals?
Yarra Valley Support Coordination (YVSC) is looking for a confident, organised and compassionateSupport Coordinatorto join our growing team in Lilydale.
This is afull-time position, working Monday to Friday from 9:00am to 5:00pm.
About the role
As a Support Coordinator with YVSC, you will manage your own caseload of NDIS participants while working closely with participants, families, nominees, providers and other professionals.
Your day-to-day responsibilities will include:
Building strong, professional relationships with participants and their support networks
Helping participants understand and implement their NDIS plans
Connecting participants with suitable providers and community supports
Coordinating services and maintaining communication between key stakeholders
Monitoring plan utilisation, budgets and service delivery
Identifying and responding to gaps, risks or barriers within a participant's support arrangements
Supporting participants through plan reassessments, changes in circumstances and NDIS processes
Preparing progress reports, supporting documentation and other correspondence for the NDIA
Maintaining clear, accurate and timely case notes and participant records
Working collaboratively with our Support Coordination, Operations and Administration teams
About you
We are looking for someone who is approachable, professional and confident managing a busy caseload while keeping participants at the centre of their work.
You will ideally have:
Previous experience inNDIS Support Coordinationor a closely related role
A strong understanding of the NDIS and participant plans
Excellent written and verbal communication skills
Excellent time-management and organisational skills, with the ability to manage competing priorities, meet deadlines and maintain oversight of a busy caseload
Confidence communicating with participants, families, providers and external stakeholders
The ability to work independently while also contributing to a supportive team environment
Strong problem-solving skills and the ability to remain calm when navigating complex situations
Strong attention to detail and a commitment to accurate, timely documentation
Confidence using online systems and case-management software
Experience working with participants with complex support needs will be highly regarded.
What you'll need
The successful applicant will be required to hold or obtain:
NDIS Worker Screening Check
Working with Children Check, where applicable
Current driver's licence
Relevant qualifications and/or demonstrated experience within disability, community services, allied health or a related field
