#### Disability Support WorkerRockhampton City, QLD ****Casual#### JobdescriptionDisabilitySupport Worker – SIL & Community ParticipationAbout the Role:We are seeking compassionate, reliable and person-centred Disability Support Workers to join our team in the Rockhampton area.You will provide 1:1 support to NDIS participants across Supported Independent Living (SIL) and Community Participation (CP) settings.
The shifts available include Day shifts for 6-8 hours, and combined Afternoon and Sleepover shifts.
You will support participants to live as independently as possible, achieve their goals, participate in their community and enjoy a safe and meaningful lifestyle.KeyResponsibilitiesProvidehigh-quality 1:1 support tailored to each participant's individual needs and goals.Assist participants with daily living activities and maintainingindependence.Providesupport within SIL homes and in the community.Support participants to attend appointments, activities, social outings and community events.Assist with meal preparation, household tasks and personal routines where required.Encourage choice, independence, dignity and activeparticipation.Followindividual support plans, NDIS requirements and organisational policies.Maintain accurate and timely progress notes and documentation.Work collaboratively with participants, families,
