Mallee Track Health & Community Service – Ouyen VIC is seeking a Rural Outreach Worker to provide person-centred, non-clinical support.
You will help people experiencing vulnerability access services, navigate systems and receive short-term assistance and referrals where needed.
The role focuses on information sharing, practical support and carers through the Carer Support Program, without counselling or ongoing case management.
