Shape the future of pharmacy practice in rural Australia
Are you a Credentialed Aged Care On-site Pharmacist (ACOP) looking to broaden your clinical practice beyond residential aged care?
Western District Health Service is offering a unique opportunity to combine aged care and hospital pharmacy practice within an innovative role that is helping redefine medicines management across rural healthcare.
This position is ideal for pharmacists seeking variety, professional growth and the opportunity to work across the full continuum of care while remaining actively involved in residential aged care services. Unlike many ACOP positions embedded within community pharmacy, our model integrates pharmacists directly into the hospital pharmacy department, creating exposure to both hospital and aged care practice.
Our hospital-led ACOP model is already attracting national attention, with outcomes to be presented at Medicines Management **** through the abstract "Implementing a hospital-led aged care onsite pharmacist (ACOP) model in rural residential care."
