An exciting chance exists for an experienced and highly motivated nurse to join the Royal North Shore Hospital Palliative Care Service as a Clinical Nurse Consultant Grade 2 (CNC2).
n
As a key member of the multidisciplinary palliative care team, the CNC will provide expert clinical care, consultation, coordination, education and leadership to support patients with life-limiting illness and their families across the continuum of care. The successful applicant will contribute to the delivery of high-quality, person-centred palliative care within the inpatient consult service and work collaboratively with medical, nursing and allied health colleagues to achieve optimal patient outcomes.

Key Responsibilities
n
n
Provide advanced clinical assessment, consultation and expert palliative care nursing practice.
n
Coordinate care for patients with complex palliative and end-of-life care needs.
n
Facilitate seamless communication and care planning across hospital, community and Palliative Care Units.
n
Support patients and families through education and advocacy.
n
Collaborate effectively within the multidisciplinary team to ensure continuity and quality of care.
n
Lead and participate in research, clinical governance and service development initiatives.
n
Provide education, mentoring and clinical support to nursing and multidisciplinary staff.
n
Contribute to data collection, service evaluation and evidence-based practice initiatives.
n

About You
n
The successful applicant will demonstrate:
n
n
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
n
Extensive clinical experience in palliative care.
n
Postgraduate qualification relevant to the specialty area at Graduate Certificate level or higher, in line with CNC Grade 2 requirements.
n
Advanced assessment, care coordination and clinical decision-making skills.
n
Excellent communication and interpersonal skills.
n
Experience working collaboratively within multidisciplinary teams.
n
Commitment to patient-centred care, quality improvement and professional development.
n

Candidates will need to meet the following criteria:
n

Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum five years full time post graduate experience with 3 years full time equivalent in Palliative Care Nursing. In addition the applicant must have an approved post graduate qualification (minimum Grad
Diploma/Certificate
level) or evidence of working towards Masters level qualification.

Demonstrated effective organisational and time management skills and the capacity to successfully manage competing priorities and meet deadlines.
n
Demonstrated ability to work independently and as part of a multidisciplinary team. Demonstrated excellent cooperative, leadership and teaching skills.
n
Excellent interpersonal oral and written communication skills and the ability to provide, information and education effectively to patients, families and carers.
n
Demonstrated advanced ability to assess, plan, initiate and manage complex cases.
