DPV Health in Melbourne's northern area is seeking a skilled SAS clinician to join the Aged Care Single Assessment System team. You will undertake client assessments, reviews and develop support plans to help clients live independently at home and in the community, across Northwestern Melbourne LGAs.
You will participate in clinical triage and case conferencing, identify risks and lead escalation as required.
