An exciting opportunity is available for an experienced Registered Nurse to join a dedicated Primary Health Care team as a Chronic Disease Integrated Team Care (ITC) Program Care Coordinator. This unique role will support clients living with complex chronic disease across Maningrida and surrounding homelands/outstations, working through the local Primary Health Care Centre.

The position is full-time and Darwin based, with a FIFO schedule of two weeks per month in Maningrida and two weeks in Darwin. Additional time in community may be required depending on client needs and community events.

What’s on Offer

- $135 - $145,000 per annum + super

- 12-month FTC with likelihood of renewal, subject to funding

- No weekend work, standard hours with no shift work

- Darwin-based with FIFO to Maningrida

You’ll work closely with individuals, families,



health practitioners and external specialists to develop personalised care plans, strengthen referral pathways and support clients to access the services they need. This is a fantastic opportunity to make a meaningful difference within a remote Aboriginal community while working as part of a committed and passionate team.

Essential Requirements

- Current AHPRA registration and practising certificate as a Registered Nurse

- Minimum 5 years' broad post-registration nursing experience

- Current Basic Life Support certificate

- Experience working in a culturally diverse setting, ideally within a remote community

- Robust clinical assessment, problem-solving and communication skills

- Understanding of primary health care and the social determinants affecting Aboriginal and Torres Strait Islander communities

- Current NT Working with Children Clearance (OCHRE Card), or ability to obtain
