Absolute Care and Health is seeking a Clinical Care Partner for a Full Time role in South Yarra, VIC. The role focuses on delivering high-quality clinical care management to clients in their own homes under the Support at Home program.

You will assess clients’ clinical needs, develop and review care plans, monitor health and coordinate supports with the Integrated Care Team. Collaboration with the Anchor Registered Nurse and other providers is essential to ensure secure, client-centred care.
