Role Description The Aged Care Case Manager is responsible for coordinating and overseeing client care plans, ensuring services meet individual needs and promote independence and quality of life. Day-to-day tasks include conducting holistic assessments, developing and reviewing support plans, coordinating allied health and support services, and monitoring outcomes against client goals.
The Case
Manager will liaise with clients, families, carers, and external providers, maintain accurate documentation, and ensure compliance with NDIS, Aged Care, and organizational standards. This is a part-time, hybrid role based in Melbourne, VIC,
with work performed both in the community, in the office, and from home as appropriate.
Qualifications
- Relevant tertiary qualification in social work, nursing, allied health, case management, or community services.
- Demonstrated experience in aged care or disability case management, including care planning and service coordination.
- Robust client assessment, advocacy, and risk management skills, with a focus on person-centered and strengths-based practice.
- Excellent communication, interpersonal, and relationship-building skills for working with clients, families, and multidisciplinary teams.
- Sound understanding of NDIS, Aged Care standards, and community care frameworks, including compliance and reporting requirements.
- Competence in using case management software and standard office applications, with accurate record-keeping and documentation skills.
- Ability to manage a diverse caseload, prioritize competing demands, and work independently in a hybrid environment.
- Cultural awareness and sensitivity, with experience supporting people from diverse cultural and linguistic backgrounds.
- Current Working with Children Check, National Police Check, and valid driver’s license are highly desirable.
