As a Graduate Social Worker at The Prince Charles Hospital, you will develop your clinical skills in a supportive hospital setting while working with experienced social workers and multidisciplinary teams. About the role
As a Graduate Social Worker at The Prince Charles Hospital, you will develop your clinical skills in a supportive hospital setting while working with experienced social workers and multidisciplinary teams. In this role, you will undertake psychosocial assessments, deliver evidence-informed interventions, support patients and families with complex needs, contribute to discharge planning and referrals, and work collaboratively to achieve patient-centred outcomes.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver high level Social Work clinical services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW professional practice standards to optimise the psychosocial outcomes, and to achieve maximum benefits from health care services to patients and their families/carers.
- Work collaboratively within a multidisciplinary team/s by effectively providing Social Work assessment and interventions to achieve optimal health outcomes for patients, families and caregivers.
- Provide education,
direction and supervision for routine tasks to less experienced staff, students and support staff, with the support of senior clinical staff.
- Participate in the identification, development, implementation and evaluation of quality and service improvement and/or research activities that enhance the delivery of social work services and promote patient centred care within an interprofessional service.
