Job Description

Peregrine Partners on the Gold Coast is seeking an experienced Clinical Case Manager and Registered Nurse to provide clinical leadership, assessments and oversight for participants receiving health and disability supports.

You will collaborate with Specialist Care Coordinators, conduct comprehensive clinical assessments, monitor changes, prepare reports and ensure protected care delivery in line with Delegation of Care requirements.
