BlueCare Care Partner role delivering person-centred home and community care across Recent Farm, Hamilton, Kedron, McDowall and surrounds. You will work with clinical teams to plan and coordinate services, visiting clients in their homes to ensure high-quality care.

Role requires IV in Health/Community Service, current Australian driver’s licence, and an insured vehicle. You’ll develop care plans, monitor outcomes, manage budgets and help clients navigate the care system.
