GRAMS in Geraldton is seeking an Aboriginal Health Practitioner to provide culturally secure primary health care, health education and ongoing support to Aboriginal clients in the clinic and in the community. You will deliver health assessments, support chronic disease management, work with GPs, nurses and visiting specialists, and contribute to health promotion and continuous quality improvement.
