About the Role

n

We’re seeking a Registered or Enrolled Nurse to join our Rockhampton Medical Centre , where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare . You’ll be part of a excellent, supportive team that values collaboration, culture, and patient outcomes.

Key Responsibilities

n

n
- Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
n
- Conduct health assessments and promote preventative care
n
- Collaborate with GPs and multidisciplinary teams, including referral coordination
n
- Manage administrative tasks related to CCM appointments
n
- Coach patients on sustainable health behaviour changes.
n




- Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
n
- Work with the practice team to grow the CCM program.
n

About You

n

To be successful in this role, you will have:

n

n
- Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN) .
n
- Prior experience in Chronic Condition Management preferred
n
- Strong communication and organisational skills
n
