Job Description
Join Our Reablement Team as a Care and Support Worker n
Are you compassionate, reliable, and committed to making a difference? We are seeking Care and Support Workers to join our Reablement Service, supporting individuals to regain independence and confidence in their own homes.
About the Role n
Contract: 26.25 hours per week
n
Service: Operates 7 days a week, year-round
n
Location: Community-based care in service users' homes
n
Purpose: Support recovery following illness, injury, or hospital discharge
Working Pattern n
4-week rolling rota
n
Morning shifts: 7:00am - 3:00pm
n
Evening shifts: 2:30pm - 10:30pm
n
Includes weekends and bank holidays
n
A full driving licence and access to a vehicle are essential
About the Service n
Our short-term, person-centred service supports individuals with disabilities or those recovering from illness. We focus on:

- Relearning daily living skills
n
- Working collaboratively with health and social care professionals
n
n
Additional support includes:
n
n
- Bridging care while long-term arrangements are made
n
- Resettlement support following time away from home
n
n
£18,438.14 to £18,731.85 per year, pro rata
n
£14,750.51 to £14,985.48 per year, pro rata
