Gold Coast Health is seeking a Clinical Nurse Consultant (Transition Care Team) to lead a dedicated team within the Transition Care Program and oversee complex client cases. Based in Southport, QLD, the role involves joint home visits, clinical leadership, and ensuring high-quality care transitions from hospital to home.

The TCP aims to provide multidisciplinary support for eligible clients over 65 (55+ for Indigenous peoples) to reduce hospital length of stay and unnecessary residential care
