Working closely with medical officers, Visiting Medical Officers (VMOs), multidisciplinary teams, carers and community providers, MCAH bridges the gap between hospital and home to deliver secure, evidence-based and compassionate care.

As part of St Vincent's Care at Home , MCAH helps patients avoid unnecessary hospital stays, promotes independence, enhances quality of life and ensures continuity of care through innovative healthcare delivery.

The Opportunity - Registered Nurse, Medical Care at Home
n
n
This is a permanent full-time, part-time or casual opportunity based in Toowoomba .
n
n
Working as part of the growing Medical Care at Home (MCAH) team, you will provide high-quality nursing care to patients receiving acute and post-acute treatment within the community.
n
n
You will work collaboratively with the MCAH Lead (Medical), Visiting Medical Officers (VMOs), St Vincent's Care facilities and a range of healthcare professionals to deliver coordinated, patient-centred care.
n
n
As a Registered Nurse, you will assess patient needs, develop tailored care plans,
provide clinical interventions and coordinate safe transitions of care.
n
n
Your work will directly contribute to reducing hospital admissions, improving patient outcomes and supporting people to recover safely in their preferred environment.
n
n

Key Responsibilities:

Clinical Care Delivery
n
Deliver high-quality, evidence-based nursing care including assessment, care planning, treatment, review and education for patients and carers.
n
n
Advanced Nursing Interventions
n
Provide nursing interventions including IV therapies, complex wound care and monitoring of patients with complex clinical conditions.
n
n
Care Planning & Coordination
n
Assess patient needs and formulate individualised care plans that promote safety, function and independence while coordinating care across services.
n
n
Patient & Family Engagement
n
Communicate effectively with patients, families and carers to support informed decision-making and active participation in care.
n
n
Clinical Escalation & Safety
n
Recognise and promptly escalate out-of-range observations and clinical deterioration in accordance with clinical guidelines and organisational protocols.
n
n
Multidisciplinary Collaboration
n
Work closely with medical officers, allied health professionals and community providers to ensure continuity of care and positive patient outcomes.
n
n
Quality & Documentation
n
Maintain accurate clinical documentation and contribute to quality improvement initiatives, case reviews, safety huddles and service excellence activities.
n
n
About You
n
n
You are a compassionate and highly skilled Registered Nurse who enjoys working autonomously while collaborating within multidisciplinary teams.
n
You are passionate about delivering patient-centred care and motivated by the opportunity to help transform healthcare delivery in the community.

You thrive in fast-paced environments,
demonstrate sound clinical judgement and are committed to achieving outstanding outcomes for patients receiving care at home.

You Will Bring
n
n
- Current registration as a Registered Nurse with AHPRA.
n
- Minimum 3 years post-graduate nursing experience.
n
- Experience caring for patients with multiple comorbidities and complex clinical needs.
n
- Community nursing experience and/or experience across acute medical, surgical or critical care settings.
n
- Demonstrated clinical knowledge and application of best-practice nursing care.
n
- Proven ability to work collaboratively within multidisciplinary healthcare teams.
n
- Commitment to evidence-based, person-centred care.
n
- Strong digital literacy skills, including the use of clinical systems and virtual healthcare platforms.
n
- Excellent communication, organisational and problem-solving skills.
n
- Ability to work independently and manage competing priorities in a time-critical environment.
n
- Commitment to ongoing learning and career development.
n
- Current unrestricted Australian Driver's Licence.
n
n
