Job Description

healthAbility's Reconnect Counsellor & Case Manager in Melbourne, VIC, AU is a part time position (0.6 FTE) within the Children, Youth and Family team. The role involves caseload management and collaborative work, co-located at the Nillumbik Youth Hub with a strong outreach focus.

The role targets young people aged 12-18 at risk of homelessness and aims to prevent homelessness while improving wellbeing. Hybrid work setup in Melbourne, VIC with ongoing outreach responsibilities.
