About the Role
This position works within the Primary Health Team with a focus on providing Chronic Disease health education and early intervention, both on an individual and group level. This will include (but not be restricted to) providing direct care to clients with chronic conditions, such as Chronic Obstructive Pulmonary Disease (COPD), Cardiovascular Disease and Diabetes and assisting them with self-management strategies. The position will provide range of primary care initiatives and direct client services, consistent with the strategic direction of PCC and in line with best practice standards.
Position Context
This position reports to the Manager Healthy Communities within the Health Services team and works within the Primary Health Team. It is also expected that this position works within the larger Primary Care Connect team, and network with external organisation.
This position is based out of PCC Shepparton office with outreach into the Strathbogie Shire as required.
Key Selection Criteria
The following criteria must be met and addressed in applications for consideration for this position:
Essential
- Registered Nurse with a Current AHPRA Registration
- Minimum 2 years of experience as a Community Health Nurse
- A thorough understanding of the principles under pinning primary health care and health promotion.
- Understanding of the clinical and social issues for people living with chronic diseases/illnesses.
- Advanced interpersonal, written and verbal communication skills, and an ability to communicate with a diverse range of people, including clients from linguistically and culturally diverse backgrounds.
- An understanding of the social determinants of health and health promoting principles.
- Professional and personal alignment with Primary Care Connect core values.
Desired
- MARAM training.
- Mental health first aid training.
- Understanding of trauma informed care and health impacts of trauma.
