Care Connect in Bella Vista, NSW, is seeking a Care Manager to join the Out of Hospital Care program on a full time, parental leave cover basis. You will assess, plan and manage services to help clients safely return home from hospital, with a focus on independence and choice.
You will build care plans, coordinate with providers, engage with clients and families, and contribute to a compassionate, values-driven team. Hybrid work from our Bella Vista office is available.
