Community Worker – Disability Services

Infinite Home Care & Disability Services Pty Ltd is a registered NDIS provider delivering person-centred disability support and community-based services across Western Sydney, NSW.

We are seeking an experienced and motivated Community Worker to join our team on a full-time basis. The successful candidate will support people with disability to achieve their individual goals, develop independence and participate in their local community.

Key Responsibilities
Provide person-centred support to NDIS participants based on their individual needs and goals.
Assist participants with community participation, social and recreational activities.
Support participants to develop independence and daily living skills.
Assist participants to access community services, appointments and activities.
Develop and implement appropriate support strategies in consultation with participants and relevant stakeholders.
Monitor participant progress and identify changes in support needs.
Maintain accurate progress notes, case notes and participant records.
Liaise with participants, families, support coordinators, health professionals and other service providers.
Identify and report incidents, risks and changes in participant circumstances.
Promote participant choice,



dignity, privacy, independence and inclusion.
Follow NDIS requirements and Infinite Home Care policies and procedures.
Work independently and as part of a professional support team.

Essential Requirements
Relevant qualification in Community Services, Disability, Individual Support or a related field.
Relevant experience working with people with disability or in community services.
Understanding of person-centred practice and NDIS principles.
Solid communication and interpersonal skills.
Positive written documentation and record-keeping skills.
Current NDIS Worker Screening Check.
Current First Aid and CPR.
Working with Children Check, where applicable.
Australian driver's licence and reliable vehicle.
Ability to work across Western Sydney.
