Coburg Aged Care Pty Ltd is seeking a compassionate Care Partner/Case Manager to coordinate Support at Home services for older Australians in Melbourne.
You'll lead care plans, assess needs, and ensure timely delivery of tailored supports in a collaborative team.
Ideal candidates will have aged care or health sector experience, robust assessment and leadership skills, and a willingness to travel.
Hybrid role with some remote coordination and client visits across Melbourne.
