Community Housing Limited is seeking an Initial Assessment & Planning Worker (Corrections) to provide person-centred housing assessment, planning and referral support for people at Fulham Correctional Centre who are at risk of homelessness or unable to sustain their tenancy on release.
You will complete housing assessments with people in custody, support access to housing pathways, provide advocacy and referrals, and build robust partnerships with Corrections Victoria and community services.
