Support Coordinator - Full Job Description

Our Support Coordinators work alongside participants to help them understand and use their NDIS plans, connect with suitable services and strengthen their confidence in navigating the NDIS and broader community.
This is a participant-focused role for someone who values independence, problem-solving, transparent communication and genuine choice and control.

About the Role
As a Support Coordinator with Dantae Support Services , you will work alongside participants to help them understand their NDIS plans, identify suitable supports and build a network that reflects their needs, goals and preferences.
Rather than making decisions for participants, your role is to provide information, explore options and support informed decision-making.
You will help participants implement their plans, connect with providers and mainstream or community services, respond to changing circumstances and progressively build their ability to manage their own supports wherever possible.
Your Role May Include
Helping participants understand their NDIS plan and funded supports
Exploring the participant's goals, priorities and preferred way of receiving support
Identifying appropriate providers and community services
Presenting participants with suitable options so they can make informed choices
Supporting participants to establish new services and supports
Helping coordinate multiple providers where supports need to work together
Building connections with mainstream and community services
Monitoring whether supports are meeting participant needs and goals
Identifying gaps or barriers within a participant's support arrangements
Supporting participants through changes in circumstances
Preparing for plan reassessments and other NDIS processes
Helping participants understand service agreements, budgets and available funding
Supporting communication between participants and their approved support network
Building participant confidence and capacity to navigate services more independently
Key Responsibilities
As part of the Dantae Support Services team, you will be expected to:
Keep the participant's goals, preferences, rights and choices at the centre of Support Coordination
Help participants understand and implement their NDIS plans
Provide clear and accessible information about available support options
Support participants to exercise genuine choice and control
Identify services that are appropriate to participant needs rather than simply directing participants to particular providers
Build and maintain effective working relationships with participants and approved stakeholders
Monitor whether support arrangements continue to meet participant needs
Identify service gaps, barriers and emerging concerns
Support participants to resolve problems within their support network
Maintain professional boundaries and manage potential conflicts of interest appropriately
Maintain accurate, timely and meaningful participant records
Follow privacy, confidentiality, consent and information-sharing requirements
Identify and appropriately elevate safeguarding or significant risk concerns
Work within applicable NDIS requirements and Dantae Support Services policies
Support participants to develop greater independence in managing their supports over time
Participant Choice & Decision-Making
Support Coordination should strengthen a participant's ability to make decisions rather than replace it.
Our Support Coordinators are expected to:
Listen to what the participant wants
Understand their goals, preferences and priorities
Explain options in a way that is clear and accessible
Allow participants appropriate time to consider choices
Respect a participant's preferred communication style
Involve family, nominees or other decision supporters where appropriate and authorised
Respect dignity of risk
Avoid unnecessary influence over participant decisions
Support participants to remain actively involved in decisions about their own lives
The participant should remain at the centre of their support network.
Building Capacity & Connections
An important part of Support Coordination is helping participants become more confident in navigating their supports.
This may involve:
Building the participant's understanding of their NDIS plan
Helping them understand how different types of funding can be used
Developing confidence communicating with providers
Supporting participants to compare service options
Building informal, community and mainstream connections
Supporting participants to identify solutions when services are not working
Developing strategies for managing future changes
Strengthening the participant's ability to coordinate more of their own supports where appropriate
The goal is not to create unnecessary dependence on the Support Coordinator. Wherever possible, the role should strengthen the participant's own skills, knowledge and confidence.
Support Coordinators may assist participants to turn an approved NDIS plan into practical supports that work in everyday life.
This can include:
Identifying the supports that need to be established
Supporting service commencement
Helping participants understand service agreements
Coordinating communication between multiple providers where required
Monitoring whether services are being delivered as expected
Identifying areas where funding may be underused or support arrangements need attention
Supporting participants to prepare for plan reassessments
Gathering relevant information from approved providers and stakeholders
Helping the participant communicate changes in their circumstances or support needs through appropriate NDIS processes
Collaboration & Stakeholder Engagement
Depending on the participant's preferences, consent and circumstances, Support Coordinators may work with:
Families and informal supports
Guardians and nominees
Disability support providers
Allied health professionals
Health services
Housing providers
Employment and education services
Government and mainstream services
Community organisations
NDIA representatives and other approved stakeholders
Effective Support Coordination requires collaboration, but the participant's privacy, consent and authority should always guide what information is shared and with whom.
Independence & Conflicts of Interest
Support Coordinators occupy a position of trust and can significantly influence the services a participant chooses.
For this reason, professional independence and transparency are important.
Support Coordinators should:
Provide participants with genuine options wherever reasonably available
Avoid pressuring participants toward a particular provider
Clearly identify and appropriately manage actual, potential or perceived conflicts of interest
Respect the participant's right to choose providers outside Dantae Support Services
Provide information that supports informed decision-making
Document significant decisions and participant preferences appropriately
Our focus is on helping participants develop a support network that works for them.
Support Coordinators may become aware of risks or concerns across a participant's broader support environment.
This may include:
Breakdown of essential supports
Significant changes in participant circumstances
Service gaps
Provider concerns
Housing instability
Safeguarding concerns
Possible abuse, neglect, exploitation or harm
Behavioural or environmental risks
Hospitalisation or major health-related changes
Situations requiring urgent coordination between services
The role requires good professional judgement, explicit documentation and appropriate escalation while respecting the participant's rights, preferences and dignity.
Documentation & Reporting
Quality Support Coordination depends on accurate records and meaningful follow-up.
Documentation may include:
Participant contact notes
Participant goals and priorities
Provider and stakeholder communication
Participant decisions and preferences
Agreed actions
Risk and safeguarding information
Service gaps and barriers
Meeting records
Plan reassessment preparation
Reports required in connection with Support Coordination services
Records should clearly demonstrate the work undertaken, the participant's involvement and the outcomes being pursued.
