Western Sydney Local Health District at Westmead Hospital seeks a Blood Transplant Post-Transplant Coordinator to provide advanced clinical expertise and autonomous care coordination for patients from Day 100 onwards, including long-term follow-up.

Lead education for patients, carers and staff, coordinate complex clinical care across settings, and drive evidence-based practice within the Blood Transplant and Cell Therapy service.
