AnglicareSA is seeking a Clinical Care Partner to support customers with complex clinical needs across Community Aged Care programs, with a primary focus on the Transitional Care Program, helping clients transition safely from hospital to home through short-term restorative care.
You will coordinate clinical care with nurses, allied health staff and service teams, provide case management and ensure timely, secure interventions aligned with customer goals.
