Aveo is seeking a Care Partner (Clinical) to advocate for residents across Aveo Northern Beaches Communities—Avalon Beach, East Lindfield, Mosman and St Ives—as they navigate home care options and plan personalised care that keeps them living independently.
You will meet with residents and families, explain funding options, develop and coordinate home care plans with GPs and allied health, monitor wellbeing, maintain records in Alayacare, and promote Aveo Care at Home through community events
