About the Role
Milestone Support Services is seeking an experienced Social Worker to join our team and support participants across our disability and community-based services.
The successful candidate will work directly with participants and their support networks to understand individual circumstances, identify support needs, coordinate appropriate services and promote positive outcomes. The role involves working with participants experiencing complex disability, psychosocial and mental health needs.
As a senior member of the team, the Social Worker will also provide guidance and mentoring to Disability Support Workers and contribute to case discussions, staff development and the ongoing quality of participant services.
Key Responsibilities
Participant Support & Case Management
Manage an assigned caseload and maintain regular communication with participants, their families and other people involved in their support.
- Meet with participants and, where appropriate, their families to understand their personal circumstances, social needs, wellbeing, challenges and existing support systems.
- Develop, implement and regularly review tailored support and case plans with participants and relevant stakeholders based on their identified needs and goals.
- Provide ongoing case management to participants experiencing complex personal, social, health and community circumstances, assisting them to work through challenges and access appropriate support.
- Provide participants with information, guidance and advocacy to assist them in making informed decisions, exercising their rights and maintaining independence and involvement in the community.
- Identify suitable health, welfare, housing, disability, employment and community-based services and arrange referrals according to each participant’s circumstances and support requirements.
- Liaise with service providers and other organisations regarding referrals, follow up on services provided and coordinate supports to promote continuity and appropriate outcomes for participants.
- Regularly review participants’ circumstances and progress, identify emerging concerns and arrange further assistance or changes to support arrangements where required.
- Assist participants during significant changes in their circumstances, including changes in services and support arrangements and transitions following hospital discharge.
- Attend case conferences, participant reviews and multidisciplinary meetings and contribute relevant information to discussions concerning participants’ needs, progress and ongoing support.
- Work with families, guardians, support coordinators, psychologists, occupational therapists, behaviour support practitioners,
health professionals and other service providers to coordinate comprehensive support for participants.
- Prepare detailed case notes, assessments, progress updates, referral documentation and other reports relating to participants and their support needs.
- Maintain complete, accurate and confidential records of participant interactions, assessments, support plans, referrals, progress and outcomes.
- Prepare and provide relevant participant information and supporting documentation for NDIS reviews, assessments and other formal service requirements.
Leadership & Supervision
As a senior Social Worker, you will also:
- Provide guidance and mentoring to Disability Support Workers in relation to participant needs and support arrangements.
- Assist staff to understand and appropriately implement individualised support plans and other participant strategies.
- Lead case discussions involving complex participant matters.
- Provide professional input during multidisciplinary discussions and participant reviews.
- Supervise and coach staff in relation to participant support and service delivery.
- Identify staff development needs and contribute to training, mentoring and onboarding.
- Promote person-centred and evidence-informed approaches to participant support.
- Support a professional team culture focused on participant wellbeing, safety and positive outcomes.
Stakeholder & Service Coordination
- Develop and maintain effective working relationships with participants, families, guardians and relevant service providers.
- Liaise with health, disability, community, housing and government services on behalf of participants where appropriate.
- Coordinate information between relevant stakeholders involved in participant support.
- Identify gaps or barriers affecting access to services and work with relevant stakeholders to address them.
- Attend stakeholder meetings and contribute to participant planning and review processes.
- Monitor participant outcomes and recommend changes to support arrangements when appropriate.
- Contribute to continuous improvement of participant services.
- Ensure work is undertaken in accordance with relevant organisational policies, professional requirements and NDIS requirements.
About You
We are looking for a qualified and experienced Social Worker who can work independently with participants while contributing positively to a multidisciplinary team.
You will have:
A Bachelor degree or higher qualification in Social Work,
or an appropriately recognised equivalent qualification.
Relevant professional experience in social work, case management, disability, mental health, community services or a related field.
Experience managing a caseload and working with individuals experiencing complex circumstances.
Demonstrated experience in assessment, case planning, advocacy, referrals and service coordination.
Experience working collaboratively with families, health professionals, community organisations and other service providers.
Strong written communication, case documentation and report-writing skills.
Ability to exercise sound professional judgement and manage sensitive and complex matters.
Strong communication, interpersonal and problem-solving skills.
Experience mentoring, supervising or providing guidance to staff.
A commitment to person-centred, ethical and respectful practice.
Understanding of the NDIS and disability services environment.
Bachelor degree or higher qualification in Social Work or an appropriately recognised equivalent qualification.
Demonstrated professional experience in social work and case management.
Experience conducting assessments and developing and reviewing support or case plans.
Demonstrated experience providing advocacy, referrals and coordinating services.
Experience working with people with complex social, psychosocial, disability or mental health needs.
Demonstrated ability to prepare professional case notes, reports and other documentation.
Demonstrated ability to work collaboratively with multidisciplinary teams and external service providers.
Demonstrated leadership, supervision, mentoring or staff-development experience.
Eligibility for or current professional registration or recognition where required.
Current NDIS Worker Screening Check.
Current Working with Children Check.
Current First Aid & CPR Certificate.
Current Driver's Licence.
Experience working within the NDIS, SIL or SDA sector.
Experience supporting people with psychosocial or intellectual disability.
Experience working with Behaviour Support Plans and risk-management strategies.
Experience working with guardians, hospitals, community organisations or government agencies.
Experience supporting participants through significant life transitions.
Knowledge of relevant disability and community service frameworks.
