About the role
Bilingual Aged Care Connector joining the Care Finder program, which aims to provide specialist and intensive support to the Care Finder program target population to understand and access aged care services and other relevant support in the community.
Key responsibilities
Provide specialist and intensive support to the Care Finder program target population
Support clients to understand and access aged care services
Connect clients with relevant community support services
Build rapport and trust with clients and stakeholders
Work with CALD communities
About you
Relevant tertiary qualifications in social work, human services, aged care, community services or health
Fluent in English and one of the following community languages: Arabic or Greek speaking
Demonstrated understanding and commitment to a person-centred approach, inclusive practice and strength-based approach
Sound knowledge of My Aged Care systems and support services that are available in the local community
Experience in working with CALD communities
Ability to build rapport and trust with clients and stakeholders
Excellent communication, negotiation, problem-solving and casework skills
Sound computer literacy
Full Driver's license and ability to use your own insured vehicle
Current Police Check
