Community Worker – Disability Services

Infinite Home Care & Disability Services Pty Ltd is a registered NDIS provider delivering person-centred disability support and community-based services across Western Sydney, NSW.

We are seeking an experienced and motivated Community Worker to join our team on a full-time basis. The successful candidate will support people with disability to achieve their individual goals, develop independence and participate in their local community.

Key Responsibilities

- Provide person-centred support to NDIS participants based on their individual needs and goals.
- Assist participants with community participation, social and recreational activities.
- Support participants to develop independence and daily living skills.
- Assist participants to access community services, appointments and activities.
- Develop and implement appropriate support strategies in consultation with participants and relevant stakeholders.
- Monitor participant progress and identify changes in support needs.
- Maintain accurate progress notes, case notes and participant records.
- Liaise with participants, families, support coordinators, health professionals and other service providers.
- Identify and report incidents, risks and changes in participant circumstances.




- Promote participant choice, dignity, privacy, independence and inclusion.
- Follow NDIS requirements and Infinite Home Care policies and procedures.
- Work independently and as part of a professional support team.

Essential Requirements

- Relevant qualification in Community Services, Disability, Individual Support or a related field.
- Relevant experience working with people with disability or in community services.
- Understanding of person-centred practice and NDIS principles.
- Strong communication and interpersonal skills.
- Good written documentation and record-keeping skills.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR.
- Working with Children Check, where applicable.
- Australian driver's licence and reliable vehicle.
- Ability to work across Western Sydney.
