Queensland Government in the Wide Bay region is seeking a Social Work clinician to deliver high level social work services to patients on the Transition Care Program. You will contribute to preventing avoidable admissions and support rehabilitation after acute care.
This role offers a competitive remuneration up to $78,680 p.a., plus up to 12.75% superannuation, annual leave loading, versatile work arrangements and professional development leave. Two permanent part-time positions, 38 hours p.f.
