Umbrella Multicultural Community Care Services Inc. Ascot| Full-time | SCHADS Homecare Level 6 ($80,186 per annum + superannuation)
The Team Leader Support Services will provide mentoring, support and coaching to Support Workers within Home Support Services. This role will ensure high-quality care and support is consistently delivered to Umbrella clients enabling them to live independently with dignity in their own homes.
What you’ll do:
- Lead, mentor and coach Support Workers to support high-quality, person-centred care.
- Conduct buddy shifts, home visits and onsite check-ins to monitor service quality and provide practical support.
- Monitor mandatory training, competencies and ongoing development requirements.
- Conduct performance appraisals and support employee development and improvement.
- Address performance, non-compliance and workforce concerns in line with organisational policies.
- Work closely with Care Partners, Clinical and Scheduling teams to resolve client care and service delivery issues.
- Support protected medication administration and follow up medication-related concerns.
- Guide Support Workers in personal care, mobility, continence support and other client-specific care needs.
- Monitor incidents, hazards and WHS risks and ensure appropriate follow-up actions are completed.
- Promote client independence, dignity, safety and wellbeing through person-centred and reablement-focused support.
What you’ll bring
- Certificate IV in Aged Care and relevant Aged Care experience.
- Demonstrated ability to coach and develop staff, build workforce capability, and foster a high-performing, inclusive culture.
- Proven experience leading cultural change and continuous improvement while ensuring compliance with aged care legislation,
quality standards and person-centred care principles.
- Trained and skilled in medical administration, manual handling and hand hygiene or the ability to attend and obtain relevant skills training as directed.
- High level of written and verbal communication skills.
- Experience working with clients and/or relevant Aged Care experience.
- Knowledge of the Aged Care Quality Standards.
- Prepared to work within Umbrella’s Philosophy, Values and Mission.
Desirable
- Clinical or healthcare sector experience is highly desirable.
- Knowledge of diversity, access, and equity issues for culturally and linguistically diverse (CaLD) and LGBTIQ + seniors.
