The Department
The Mobile Assessment & Treatment Service (MATS) is one of the Health of Older People (HoP) teams, sitting within the Home Acute & Community (HAC) program of Bayside Health Alfred Care Group (ACG).
It is a multi-disciplinary team consisting of medical, nursing, allied health, pharmacy and admin roles, overseen clinically by Geriatricians.
The service provides rapid clinical in-reach to residents living in residential aged care Homes [RACH’s] and the community, who are at risk of presenting to emergency departments for reasons that could be managed outside of the hospital setting. The MATS patient cohorts are the frail and elderly, or those with severe disability in residential settings. Often MATS patients have a range of chronic conditions and conditions of ageing.
The Role
- Receive and triage incoming referrals to MATS from the community (eg: from RACH’s, GP’s, AV, VVED), and from within our ACG (Alfred & Sandringham Hospital ED’s, and frequent In-patient referrers such as GMU)
- Conduct Initial needs identification and prioritisation assessments of all new referrals
- Manage the allocation of workload for the outreach teams, according to the acuity of referrals and the complexity of care
- Respond to hospital and ED patient flow pressures, by undertaking/allocating fast-track assessments and patient transfers to MATS
- Care Coordination of current MATS patients to ensure timely and quality care, early identification and escalation of clinical deterioration and formation of plans that support patient centre care and decision making. Strong care coordination also supports patient flow through the service to meet demand pressures
- Outreaching visits to provide direct care as required
- Be actively involved with service quality improvement initiatives by way of portfolio in areas of interest
- The successful applicant will demonstrate the ability to work in a dynamic environment, problem solve, negotiate, and collaborate to achieve the best available outcomes for our MATS patients.
Experience and Qualifications Required
Essential
- Current professional registration with the Nursing and Midwifery Board of Australia via AHPRA
- Five years or more of post graduate clinical experience
- Sound clinical knowledge of complex health and medical conditions
Desirable
- Postgraduate qualifications in gerontology nursing and/or palliative care nursing; Emergency or critical care nursing; or other relevant field.
- Experience with clinical triage
- Clinical competence/credentialled in catheterisation for male/female IDC’s & SPC’s; IV Cannulation; Gastrostomy Tube management.
- Recent clinical experience in Emergency and/or General Medicine, Acute aged care or Residential Aged Care nursing.
