AMITA Health Care Group is seeking a caring, reliable and motivated Disability Support Worker to provide individualised support to an NDIS participant.

The role will primarily involve community access, meal preparation and assistance with everyday activities, with a focus on supporting the participant to maintain independence, choice and control.

Key responsibilities include:
Supporting the participant with community access and social activities
Assisting with shopping, appointments and other community-based activities
Meal planning and preparation
Supporting the participant with daily living tasks as required
Encouraging independence and participation in activities of the participant’s choice
Providing companionship and positive social engagement
Supporting the participant to work towards their NDIS goals
Providing transport where required
Maintaining qualified boundaries and respecting the participant’s privacy, dignity and choices
Completing progress notes and other required documentation
Following the participant’s support plan, risk assessments and organisational policies

To be considered, applicants should have:
Previous disability or community support experience preferred




Certificate III in Individual Support, Disability, Community Services or similar is desirable
Current NDIS Worker Screening Check
Working With Children Check, where required
Current First Aid and CPR
Valid Australian driver’s licence
Reliable vehicle with appropriate insurance
Valuable communication and interpersonal skills
Ability to work independently and reliably
A respectful, patient and person-centred approach
