Best of Homecare in Melbourne is seeking a proactive Support Coordinator to assist participants to navigate the NDIS, develop plans and connect them with appropriate services. You will monitor progress, liaise with providers and advocate for participant needs to maximise outcomes and independence.

Experience in disability or aged care, solid organisational skills and knowledge of NDIS rules are essential, with ability to work collaboratively in a local community network.
