Practice Nurse | Chronic Condition Management | Rockhampton

About the Role

We’re seeking a Registered or Enrolled Nurse to join our Rockhampton Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a excellent, supportive team that values collaboration, culture, and patient outcomes.

Key Responsibilities
Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
Conduct health assessments and promote preventative care
Collaborate with GPs and multidisciplinary teams, including referral coordination
Manage administrative tasks related to CCM appointments
Coach patients on sustainable health behaviour changes.




Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
Work with the practice team to grow the CCM program.

About You

To be successful in this role, you will have:
Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
Prior experience in Chronic Condition Management preferred
Strong communication and organisational skills
