Australian Unity is seeking a Clinical Care Partner to support protected, coordinated care for customers with complex or changing needs.
This permanent full-time role provides clinical leadership, escalation management and decision support within the Neighbourhood Care Partnering model.
You will collaborate with Care Partners and the multidisciplinary team to manage clinical risk, support transitions of care and ensure timely, appropriate interventions.
