IUIH Institute for Urban Indigenous Health is seeking a Care Coordinator – MATSICHS Social Health to provide coordination of supports for Aboriginal and Torres Strait Islander clients with complex social and mental health needs.
You will carry out intake, screening, and non-clinical assessments, and collaborate with clinical staff for higher-level work.
You will manage a caseload, support care plan development, and may work across Caboolture, Morayfield, and Strathpine as required.
