Catholic Healthcare is seeking a Care Partner to provide relationship-centred care at home across the Northern Sydney area. The role will be office-based at Hunters Hill with visits to clients in the Northern Sydney region.

You will manage care plans, coordinate with clinical and allied teams, and ensure accurate billing and documentation while aligning services with aged care standards. This permanent full time role offers a competitive hourly rate plus salary packaging.
