HammondCare Nowra is seeking a Care Partner to deliver and develop tailored Care Plans for each client, overseeing ongoing assessments and ensuring services meet funding and budget expectations. The role is full time, Monday to Friday at the HammondCare at Home Nowra Office, supporting growth.

You will manage care plans with stakeholders, support client KPIs, maintain up-to-date plans, and provide skilled day-to-day care delivery.
