North Coast Aboriginal Corporation for Community Health (NCACCH), now trading as Manngoor Dja Aboriginal Health Services is an Aboriginal Community Controlled Health Organisation committed to improving the health and wellbeing of Aboriginal and Torres Strait Islander peoples across the Sunshine Coast and Gympie regions.

Social Worker
to join our Gympie team and make a difference in the lives of our community.
About the Role
The NCACCH Social Worker - Social Emotional Wellbeing (SEWB) is part of the of the Health Services team and reports directly to the General Manager – Community & Aged Care Services.
The primary purpose of this position, in accordance with the NCACCH SEWB Program and clients' assessed needs, is to ensure NCACCH clients are supported with appropriate mental health intervention practices, case management and referral pathways. They are responsible for providing line management for the SEWB Officer and overseeing their development within the sector. The Social Worker – SEWB leads the care coordination and operates within a team environment with the line management responsibilities the aim of improving the health and wellbeing of Aboriginal and Torres Strait Islander people through the provision of comprehensive and culturally valid primary health care.
About You
We are looking for someone who:
Is passionate about supporting Aboriginal and Torres Strait Islander communities
Preferably is an Accredited Mental Health Social Work or has a minimum of 3 years' skilled experience of social work, preferable within Mental Health
Has previous experience within an Aboriginal and Torres Strait Islander Community Controlled Health Service (ACCHO) or similar
Demonstrated experience providing follow-up care services for patients and families who are identified as being at risk and/or have a chronic condition and/or mental health condition
Experience supervising and coordinating teams with the ability to plan, coordinate and delegate workloads to meet business objectives
Well-developed project management skills, including the ability to deliver under pressure, meet deadlines and manage competing work priorities
Is person centred and can work with respect, empathy, and cultural sensitivity
Is committed to teamwork and building strong community relationships
