For people experiencing homelessness alongside significant mental health challenges, navigating housing, hospital discharge and community support can feel overwhelming.

As our Homelessness Mental Health Housing In‐Reach Caseworker, you'll play a vital role in helping people move from crisis and uncertainty towards greater stability, connection and hope.
Based within our Specialist Homelessness Services team at our Wyong Hub, you'll work at the intersection of mental health and housing, supporting stronger pathways for people experiencing homelessness or at risk of homelessness — including people transitioning from inpatient mental health facilities.
You'll be a connector, advocate, educator and trusted source of expertise.
You'll work alongside our SHS team to build their confidence and capability in supporting people with significant mental health needs.
You'll provide training, mentoring and secondary consultation, sharing your knowledge of mental health systems, discharge processes, referral pathways, community mental health supports, CLS, NDIS linkages and psychosocial services.
You'll also build solid relationships across the local service system — connecting with inpatient mental health units, community mental health services, the LHD, housing providers, government and community organisations to strengthen referral pathways and support coordinated discharge planning.
Most importantly, you'll keep the person at the centre.
You'll provide a flexible and responsive range of support, including assessment, short‐term casework, case planning, service coordination, outreach, active holding, advocacy and referral support.
You'll adapt your approach to meet people where they're at,



including engaging with people while they're in mental health facilities.
This role is ideal for someone who understands that meaningful change rarely happens in isolation.
You'll develop and maintain strong local service networks, identify opportunities for collaboration and help create clearer pathways between homelessness, mental health and housing services.
Your relationships and system knowledge will help ensure people aren't lost between services when they need support most.
You'll also have the opportunity to represent Uniting at local working groups and inter‐agency forums, contributing to stronger, more connected services across the Central Coast.
Your Qualifications & Experience
We're looking for someone who brings professional expertise, compassion and a genuine commitment to improving outcomes for people experiencing homelessness and mental health challenges.
You'll bring:
Tertiary qualifications in social sciences and relevant experience working with people experiencing complex mental health.
A strong understanding of the intersection between homelessness and mental health.
The ability to engage people experiencing crisis using a client‐centred, strengths‐based approach.
Strong assessment, communication, problem‐solving and service coordination skills.
The ability to build productive relationships with both internal and external stakeholders.
Confidence working independently while also being an engaged and collaborative team member.
Sound judgement when determining the level and urgency of support required.
The flexibility to work at times and locations that best meet client needs.
A current driver's licence and willingness to use a vehicle for work‐related travel where required.
