The Virtual Rehabilitation Ward at Flinders Medical Centre provides care to people with complex medical, functional, and psychosocial needs in their own home - in place of an extended or avoidable hospital admission.
About the role:
Reporting to the Manager, this role is responsible for providing specialist social work assessment, intervention and care planning to people with complex medical, functional and psychosocial needs - supporting them to remain in their own home in lieu of an extended or avoidable hospital admission.
Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
Independently assessing clients' abilities, problems and needs using a variety of formal and informal assessments as needed.
Communicating with clients' family, carers and other service providers.
Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to clients.
Initiating, developing and contributing to educational activities within the team.
About you:
A degree or equivalent in Social Work,



which gives eligibility for full membership of the Australian Association of Social Workers Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs.
Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
Experience providing social work services to people in a hospital setting.
Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards.
