We're looking for an experienced, hands-on
Team Leader
to lead a team of support workers in a
family home
, working alongside
nursing staff
. The team supports participants living with an
Acquired Brain Injury (ABI)
who need
high intensity clinical support
.

About the Role
This is a
full time
,
Toowoomba-based
position classified at
SCHADS Award Level 3, Pay Point 1
. It combines leading your team in the family home with time in the office for supervision, meetings and compliance.
Your week:
4 shifts per week:
6:00am – 2:00pm (in the home, leading and delivering support)
1 office day per week:
9:00am – 5:00pm (rostering, supervision, compliance and planning)
On-call roster:
you'll share the on-call roster with other Team Leaders. Outside business hours, you'll give your team support and advice, help fill shifts and manage incidents.
You'll report to the Area Manager. You'll be responsible for the daily quality of care and for making sure participants with ABI and complex health needs get safe, skilled, person‐centred support in their family home.
Key responsibilities include:
High Intensity Support Delivery & Oversight
Delivering and overseeing high intensity daily personal activities in line with each participant's support plan, health practitioner directions and the NDIS Practice Standards (HIDPA Module). Depending on participant needs, this may include:
Enteral feeding and management
Tracheostomy and ventilator management
Urinary catheter management
Complex wound care
Epilepsy and seizure support
Supporting participants with the cognitive, physical, communication and behavioural effects of ABI, including memory, fatigue, emotional regulation and changes in routine
Maintaining compliance and training: making sure every worker is trained and assessed as competent against the High Intensity Support Skills Descriptors (HISSD) before delivering support, and keeping training registers audit‐ready
Monitoring participants for changes in health and escalating promptly to nursing staff, health practitioners and management
Day‐to‐day supervision,
coaching and role modelling for your team of Lifestyle Support Workers
Working collaboratively with nursing staff to make sure care is coordinated and consistent
Running monthly formal supervisions and regular team meetings
Setting clear expectations and managing performance when needed
Managing your team's roster so every shift is covered, in line with the SCHADS Award and Fair Work requirements
Participating in the on‐call roster and leading debriefs after critical incidents
Compliance & Collaboration
Making sure shift notes, incident reports and documentation are completed on time and to a high standard
Working with participants, families, support coordinators, GPs, nurses and allied health professionals across Toowoomba, including reviews of high intensity support plans
Working with the family to plan weekly routines, appointments and activities, with participants at the centre
This is not a "rosters and paperwork" role. You'll be in the home most of the week, leading by example and showing your team what excellent ABI and high intensity support looks like.

Who We're Looking For
This role will suit someone who:
Has experience as a disability support worker and is ready to step into, or continue in, a leadership role
Understands ABI and how it can affect a person's cognition, behaviour, mobility and independence
Is calm, emotionally resilient and confident making sound judgements under pressure
Is comfortable working respectfully and professionally within a family home, building positive relationships with family members
Enjoys coaching others and building a capable,
consistent team
Values collaboration and openness, and brings curiosity and compassion to their work
Communicates clearly and professionally with participants, families, nursing staff, health practitioners and colleagues
Can advocate for participants while staying objective
Certificate IV or Diploma in Community Services, Disability or Nursing
Demonstrated people leadership experience
Demonstrated experience delivering or overseeing high intensity or complex health‐related supports for people with disability
Completed training and assessed competency in relevant high intensity supports, or willingness to complete it before delivering support
Working knowledge of the NDIS Practice Standards (including the HIDPA Module), the HISSD and the NDIS Workforce Capability Framework
Working knowledge of NIISQ
Current NDIS Worker Screening Clearance (Yellow Card)
Current Blue Card (Working with Children Check)
Current First Aid and CPR
Current full Australian driver licence, with a registered and insured vehicle
Ability to travel within Toowoomba and the surrounding area
Willingness to take part in the on‐call roster and ongoing refresher training and reassessment
Previous experience supporting people with Acquired Brain Injury and Clinical Care
Medication administration training and experience
Experience with rostering systems and team supervision
Knowledge of local Toowoomba health, allied health and community services
