The unit is responsible for maintaining and optimising the portfolios of Refer Your Patient incorporating Clinical Prioritisation Criteria (CPC), Smart Referrals and the Referral Intake Hub. These portfolios are integral to the ongoing monitoring of referral management demand and capacity across GCHHS.
